If, at any time, a claim has been rejected, my tendency has been to write a letter to the insurer, the doctor and my employer (who ultimately selected the insurer) saying essentially &quot;Fix this.&quot;  

There have been precisely two occasions when I have had to repeat my request.  On one of these two, the doctor&#039;s secretary pitched in (they wanted to get paid, after all, and I sure the hell wasn&#039;t going to be the one paying) and wrote a scathing letter to the insurer.  On the other occasion, I showed up in the insurer&#039;s lobby and advised them that the problem was to be fixed immediately.  They were very apologetic, because the issue had truly been a case of paperwork sliding between the cracks.</description>
